While eye tracking research in conventional radiography has flourished over the past decades, the number of eye tracking studies that looked at multislice images lags behind. A possible reason for the lack of studies in this area might be that the eye tracking methodology used in the context of conventional radiography cannot be applied one-on-one to volumetric imaging material. Challenges associated with eye tracking in volumetric imaging are particularly associated with the selection of stimulus material, the detection of events in the eye tracking data, the calculation of meaningful eye tracking parameters, and the reporting of abnormalities. However, all of these challenges can be addressed in the design of the experiment. If this is done, eye tracking studies using volumetric imaging material offer almost unlimited opportunity for perception research and are highly relevant as the number of volumetric images that are acquired and interpreted is rising.

When two images are perfectly aligned, even subtle differences are readily detected when the images are “toggled” back and forth in the same location. However, substantial changes between two photographs can be missed if the images are misaligned (“change blindness”). Nevertheless, recent work from our lab, testing nonradiologists, suggests that toggling misaligned photographs leads to superior performance compared to side-by-side viewing (SBS). In order to determine if a benefit of toggling misaligned images may be observed in clinical mammography, we developed an image toggling technique where pairs of new and prior breast imaging exam images could be efficiently toggled back and forth. Twenty-three radiologists read 10 mammograms evenly divided in toggle and SBS modes. The toggle mode led to a 6-s benefit in reaching a decision [t(22)=5.11, p<.05]. The toggle viewing mode also led to a 5% improvement in diagnostic accuracy, though in our small sample this effect was not statistically reliable. Time savings were found even though successive mammograms were not perfectly aligned. Given the ever-increasing caseload for radiologists, this simple manipulation of how the images are viewed could save valuable time in clinical practice, allowing radiologists to read more cases or spend more time on difficult cases.

Discussions of percent breast density (PD) and breast cancer risk implicitly assume that visual assessments of PD are comparable between vendors despite differences in technology and display algorithms. This study examines the extent to which visual assessments of PD differ between mammograms acquired from two vendors. Pairs of “for presentation” digital mammography images were obtained from two mammography units for 146 women who had a screening mammogram on one vendor unit followed by a diagnostic mammogram on a different vendor unit. Four radiologists independently visually assessed PD from single left mediolateral oblique view images from the two vendors. Analysis of variance, intra-class correlation coefficients (ICC), scatter plots, and Bland–Altman plots were used to evaluate PD assessments between vendors. The mean radiologist PD for each image was used as a consensus PD measure. Overall agreement of the PD assessments was excellent between the two vendors with an ICC of 0.95 (95% confidence interval: 0.93 to 0.97). Bland–Altman plots demonstrated narrow upper and lower limits of agreement between the vendors with only a small bias (2.3 percentage points). The results of this study support the assumption that visual assessment of PD is consistent across mammography vendors despite vendor-specific appearances of “for presentation” images.

Expertise with encoding material has been shown to aid long-term memory for that material. It is not clear how relevant this expertise is for image memorability (e.g., radiologists’ memory for radiographs), and how robust over time. In two studies, we tested scene memory using a standard long-term memory paradigm. One compared the performance of radiologists to naïve observers on two image sets, chest radiographs and everyday scenes, and the other radiologists’ memory with immediate as opposed to delayed recognition tests using musculoskeletal radiographs and forest scenes. Radiologists’ memory was better than novices for images of expertise but no different for everyday scenes. With the heterogeneity of image sets equated, radiologists’ expertise with radiographs afforded them better memory for the musculoskeletal radiographs than forest scenes. Enhanced memory for images of expertise disappeared over time, resulting in chance level performance for both image sets after weeks of delay. Expertise with the material is important for visual memorability but not to the same extent as idiosyncratic detail and variability of the image set. Similar memory decline with time for images of expertise as for everyday scenes further suggests that extended familiarity with an image is not a robust factor for visual memorability.

The goal of this research is to demonstrate that teaching healthcare trainees a formal search or scan pattern for evaluation of the lungs improves their ability to identify pulmonary nodules on chest radiographs (CXRs). A group of physician assistant trainees were randomly assigned to control and experimental groups. Each group was shown two sets of CXRs, each set with a nodule prevalence of approximately 50%. The experimental group received search pattern training between case sets, whereas the control group did not. Both groups were asked to mark nodules when present and indicate their diagnostic confidence. Subject performance at nodule detection was quantified using changes in area under the localization receiver operating characteristic curve (ΔAUC). There was no significant improvement in performance between case sets for the control group. There was a significant improvement in subject performance after training for the experimental group, ΔAUC=0.1539, p=0.0012. These results demonstrate that teaching a search pattern to trainees improves their ability to identify nodules and decreases the number of perceptual errors in nodule identification, and suggest that our knowledge of medical image perception may be used to develop rational tools for the education of healthcare trainees.

Three-dimensional (3-D) imaging has been intensively studied in the past few decades. Depth information is an important added value of 3-D systems over two-dimensional systems. Special focuses were devoted to the development of stereo matching methods for the generation of disparity maps (i.e., depth information within a 3-D scene). Dedicated frameworks were designed to evaluate and rank the performance of different stereo matching methods but never considering x-ray medical images. Yet, 3-D x-ray acquisition systems and 3-D medical displays have already been introduced into the diagnostic market. To access the depth information within x-ray stereoscopic images, computing accurate disparity maps is essential. We aimed at developing a framework dedicated to x-ray stereoscopic breast images used to evaluate and rank several stereo matching methods. A multiresolution pyramid optimization approach was integrated to the framework to increase the accuracy and the efficiency of the stereo matching techniques. Finally, a metric was designed to score the results of the stereo matching compared with the ground truth. Eight methods were evaluated and four of them [locally scaled sum of absolute differences (LSAD), zero mean sum of absolute differences, zero mean sum of squared differences, and locally scaled mean sum of squared differences] appeared to perform equally good with an average error score of 0.04 (0 is the perfect matching). LSAD was selected for generating the disparity maps.

We propose an implementation of the Hotelling observer that can be applied to the optimization of linear image reconstruction algorithms in digital breast tomosynthesis. The method is based on considering information within a specific region of interest, and it is applied to the optimization of algorithms for detectability of microcalcifications. Several linear algorithms are considered: simple back-projection, filtered back-projection, back-projection filtration, and Λ-tomography. The optimized algorithms are then evaluated through the reconstruction of phantom data. The method appears robust across algorithms and parameters and leads to the generation of algorithm implementations which subjectively appear optimized for the task of interest.

X-ray medical imaging is increasingly becoming three-dimensional (3-D). The dose to the population and its management are of special concern in computed tomography (CT). Task-based methods with model observers to assess the dose-image quality trade-off are promising tools, but they still need to be validated for real volumetric images. The purpose of the present work is to evaluate anthropomorphic model observers in 3-D detection tasks for low-contrast CT images. We scanned a low-contrast phantom containing four types of signals at three dose levels and used two reconstruction algorithms. We implemented a multislice model observer based on the channelized Hotelling observer (msCHO) with anthropomorphic channels and investigated different internal noise methods. We found a good correlation for all tested model observers. These results suggest that the msCHO can be used as a relevant task-based method to evaluate low-contrast detection for CT and optimize scan protocols to lower dose in an efficient way.

Task-based medical image quality is typically measured by the degree to which a human observer can perform a diagnostic task in a psychophysical human observer study. During a typical study, an observer is asked to provide a numerical score quantifying his confidence as to whether an image contains a diagnostic marker or not. Such scores are then used to measure the observers’ diagnostic accuracy, summarized by the receiver operating characteristic (ROC) curve and the area under ROC curve. These types of human studies are difficult to arrange, costly, and time consuming. In addition, human observers involved in this type of study should be experts on the image genre to avoid inconsistent scoring through the lengthy study. In two-alternative forced choice (2AFC) studies, known to be faster, two images are compared simultaneously and a single indicator is given. Unfortunately, the 2AFC approach cannot lead to a full ROC curve or a set of image scores. The aim of this work is to propose a methodology in which multiple rounds of the 2AFC studies are used to re-estimate an image confidence score (a.k.a. rating, ranking) and generate the full ROC curve. In the proposed approach, we treat image confidence score as an unknown rating that needs to be estimated and 2AFC as a two-player match game. To achieve this, we use the ELO rating system, which is used for calculating the relative skill levels of players in competitor-versus-competitor games such as chess. The proposed methodology is not limited to ELO, and other rating methods such as TrueSkill™, Chessmetrics, or Glicko can be also used. The presented results, using simulated data, indicate that a full ROC curve can be recovered using several rounds of 2AFC studies and that the best pairing strategy starts with the first round of pairing abnormal versus normal images (as in the classical 2AFC approach) followed by a number of rounds using random pairing. In addition, the proposed method was tested in a pilot human observer study. These pilot results indicate that three to five rounds of 2AFC studies require less human observer time than a full scoring study and that the re-estimated ROC curves and associated area under ROC curve values have high statistical agreement with the full scoring study.

Medical images and videos are now increasingly part of modern telecommunication applications, including telemedicinal applications, favored by advancements in video compression and communication technologies. Medical video quality evaluation is essential for modern applications since compression and transmission processes often compromise the video quality. Several state-of-the-art video quality metrics used for quality evaluation assess the perceptual quality of the video. For a medical video, assessing quality in terms of “diagnostic” value rather than “perceptual” quality is more important. We present a diagnostic-quality–oriented video quality metric for quality evaluation of cardiac ultrasound videos. Cardiac ultrasound videos are characterized by rapid repetitive cardiac motions and distinct structural information characteristics that are explored by the proposed metric. Cardiac ultrasound video quality index, the proposed metric, is a full reference metric and uses the motion and edge information of the cardiac ultrasound video to evaluate the video quality. The metric was evaluated for its performance in approximating the quality of cardiac ultrasound videos by testing its correlation with the subjective scores of medical experts. The results of our tests showed that the metric has high correlation with medical expert opinions and in several cases outperforms the state-of-the-art video quality metrics considered in our tests.

Contrast enhancement is a key component of computed tomography (CT) imaging and offers opportunities for optimization. The design and optimization of techniques, however, require orchestration with the scan parameters and, further, a methodology to relate contrast enhancement and injection function. We used such a methodology to develop a method, the analytical inverse method, to predict the required injection function to achieve a desired contrast enhancement in a given organ by incorporation of a physiologically based compartmental model. The method was evaluated across 32 different target contrast enhancement functions for aorta, kidney, stomach, small intestine, and liver. The results exhibited that the analytical inverse method offers accurate performance with error in the range of 10% deviation between the predicted and desired organ enhancement curves. However, this method is incapable of predicting the injection function based on the liver enhancement. The findings of this study can be useful in optimizing contrast medium injection function as well as scan timing to provide more consistency in the way contrast-enhanced CT examinations are performed. To our knowledge, this work is one of the first attempts to predict the contrast material injection function for a desired organ enhancement curve.

Fluence field modulated (FFM) CT allows for improvements in image quality and dose reduction. To date, only one-dimensional modulators have been proposed, as the extension to two-dimensional (2-D) modulation is difficult with solid-metal attenuation-based fluence field modulated designs. This work proposes to use liquid and gas to attenuate the x-ray beam, as unlike solids, these materials can be arranged allowing for 2-D fluence modulation. The thickness of liquid and the pressure for a given path length of gas were determined that provided the same attenuation as 30 cm of soft tissue at 80, 100, 120, and 140 kV. Liquid iodine, zinc chloride, cerium chloride, erbium oxide, iron oxide, and gadolinium chloride were studied. Gaseous xenon, uranium hexafluoride, tungsten hexafluoride, and nickel tetracarbonyl were also studied. Additionally, we performed a proof-of-concept experiment using a 96 cell array in which the liquid thickness in each cell was adjusted manually. Liquid thickness varied as a function of kV and chemical composition, with erbium oxide allowing for the smallest thickness. For the gases, tungsten hexaflouride required the smallest pressure to compensate for 30 cm of soft tissue. The 96 cell iodine attenuator allowed for a reduction in both dynamic range to the detector and scatter-to-primary ratio. For both liquids and gases, when k-edges were located within the diagnostic energy range used for imaging, the mean beam energy exhibited the smallest change with compensation amount. The thickness of liquids and the gas pressure seem logistically implementable within the space constraints of C-arm-based cone beam CT (CBCT) and diagnostic CT systems. The gas pressures also seem logistically implementable within the space and tube loading constraints of CBCT and diagnostic CT systems.

C-Arm CT three-dimensional (3-D) digital subtraction angiography (DSA) reconstructions cannot provide temporal information to radiologists. Four-dimensional (4-D) DSA provides a time series of 3-D volumes utilizing temporal dynamics in the two-dimensional (2-D) projections using a constraining image reconstruction approach. Volumetric limiting spatial resolution (VLSR) of 4-D DSA is quantified and compared to a 3-D DSA. The effects of varying 4-D DSA parameters of 2-D projection blurring kernel size and threshold of the 3-D DSA (constraining image) of an in silico phantom (ISPH) and physical phantom (PPH) were investigated. The PPH consisted of a 76-micron tungsten wire. An 8-s/248-frame/198-deg scan protocol acquired the projection data. VLSR was determined from MTF curves generated from each 2-D transverse slice of every (248) 4-D temporal frame. 4-D DSA results for PPH and ISPH were compared to the 3-D DSA. 3-D DSA analysis resulted in a VLSR of 2.28 and 1.69  lp/mm for ISPH and PPH, respectively. Kernel sizes of either 10×10 or 20×20  pixels with a 3-D DSA constraining image threshold of 10% provided 4-D DSA VLSR nearest to the 3-D DSA. 4-D DSA yielded 2.21 and 1.67  lp/mm with a percent error of 3.1 and 1.2% for ISPH and PPH, respectively, as compared to 3-D DSA. This research indicates 4-D DSA is capable of retaining the resolution of 3-D DSA.

This work focuses on volume estimation of “multidensity” lung nodules in a phantom computed tomography study. Eight objects were manufactured by enclosing spherical cores within larger spheres of double the diameter but with a different density. Different combinations of outer-shell/inner-core diameters and densities were created. The nodules were placed within an anthropomorphic phantom and scanned with various acquisition and reconstruction parameters. The volumes of the entire multidensity object as well as the inner core of the object were estimated using a model-based volume estimator. Results showed percent volume bias across all nodules and imaging protocols with slice thicknesses <5  mm ranging from −5.1% to 6.6% for the entire object (standard deviation ranged from 1.5% to 7.6%), and within −12.6% to 5.7% for the inner-core measurement (standard deviation ranged from 2.0% to 17.7%). Overall, the estimation error was larger for the inner-core measurements, which was expected due to the smaller size of the core. Reconstructed slice thickness was found to substantially affect volumetric error for both tasks; exposure and reconstruction kernel were not. These findings provide information for understanding uncertainty in volumetry of nodules that include multiple densities such as ground glass opacities with a solid component.

A scatter imaging technique for the differentiation of cancerous and healthy breast tissue in a heterogeneous sample is introduced in this work. Such a technique has potential utility in intraoperative margin assessment during lumpectomy procedures. In this work, we investigate the feasibility of the imaging method for tumor classification using Monte Carlo simulations and physical experiments. The coded aperture coherent scatter spectral imaging technique was used to reconstruct three-dimensional (3-D) images of breast tissue samples acquired through a single-position snapshot acquisition, without rotation as is required in coherent scatter computed tomography. We perform a quantitative assessment of the accuracy of the cancerous voxel classification using Monte Carlo simulations of the imaging system; describe our experimental implementation of coded aperture scatter imaging; show the reconstructed images of the breast tissue samples; and present segmentations of the 3-D images in order to identify the cancerous and healthy tissue in the samples. From the Monte Carlo simulations, we find that coded aperture scatter imaging is able to reconstruct images of the samples and identify the distribution of cancerous and healthy tissues (i.e., fibroglandular, adipose, or a mix of the two) inside them with a cancerous voxel identification sensitivity, specificity, and accuracy of 92.4%, 91.9%, and 92.0%, respectively. From the experimental results, we find that the technique is able to identify cancerous and healthy tissue samples and reconstruct differential coherent scatter cross sections that are highly correlated with those measured by other groups using x-ray diffraction. Coded aperture scatter imaging has the potential to provide scatter images that automatically differentiate cancerous and healthy tissue inside samples within a time on the order of a minute per slice.

In low-resolution phase contrast magnetic resonance angiography, the maximum intensity projected channel images will be blurred with consequent loss of vascular details. The channel images are enhanced using a stabilized deblurring filter, applied to each channel prior to combining the individual channel images. The stabilized deblurring is obtained by the addition of a nonlocal regularization term to the reverse heat equation, referred to as nonlocally stabilized reverse diffusion filter. Unlike reverse diffusion filter, which is highly unstable and blows up noise, nonlocal stabilization enhances intensity projected parallel images uniformly. Application to multichannel vessel enhancement is illustrated using both volunteer data and simulated multichannel angiograms. Robustness of the filter applied to volunteer datasets is shown using statistically validated improvement in flow quantification. Improved performance in terms of preserving vascular structures and phased array reconstruction in both simulated and real data is demonstrated using structureness measure and contrast ratio.

Pericardial fat volume (PFV) is emerging as an important parameter for cardiovascular risk stratification. We propose a hybrid approach for automated PFV quantification from water/fat-resolved whole-heart noncontrast coronary magnetic resonance angiography (MRA). Ten coronary MRA datasets were acquired. Image reconstruction and phase-based water-fat separation were conducted offline. Our proposed algorithm first roughly segments the heart region on the original image using a simplified atlas-based segmentation with four cases in the atlas. To get exact boundaries of pericardial fat, a three-dimensional graph-based segmentation is used to generate fat and nonfat components on the fat-only image. The algorithm then selects the components that represent pericardial fat. We validated the quantification results on the remaining six subjects and compared them with manual quantifications by an expert reader. The PFV quantified by our algorithm was 62.78±27.85  cm³, compared to 58.66±27.05  cm³ by the expert reader, which were not significantly different (p=0.47) and showed excellent correlation (R=0.89,p<0.01). The mean absolute difference in PFV between the algorithm and the expert reader was 9.9±8.2  cm³. The mean value of the paired differences was −4.13  cm³ (95% confidence interval: −14.47 to 6.21). The mean Dice coefficient of pericardial fat voxels was 0.82±0.06. Our approach may potentially be applied in a clinical setting, allowing for accurate magnetic resonance imaging (MRI)-based PFV quantification without tedious manual tracing.

We propose an active learning (AL) approach for prostate segmentation from magnetic resonance images. Our label query strategy is inspired from the principles of visual saliency that have similar considerations for choosing the most salient region. These similarities are encoded in a graph using classification maps and low-level features. Random walks are used to identify the most informative node, which is equivalent to the label query sample in AL. To reduce computation time, a volume of interest (VOI) is identified and all subsequent analysis, such as probability map generation using semisupervised random forest classifiers and label query, is restricted to this VOI. The negative log-likelihood of the probability maps serves as the penalty cost in a second-order Markov random field cost function, which is optimized using graph cuts for prostate segmentation. Experimental results on the Medical Image Computing and Computer Assisted Intervention (MICCAI) 2012 prostate segmentation challenge show the superior performance of our approach to conventional methods using fully supervised learning.

A detailed analysis of the quaternion generic Jacobi-Fourier moments (QGJFMs) for color image description is presented. In order to reach numerical stability, a recursive approach is used during the computation of the generic Jacobi radial polynomials. Moreover, a search criterion is performed to establish the best values for the parameters α and β of the radial Jacobi polynomial families. Additionally, a polar pixel approach is taken into account to increase the numerical accuracy in the calculation of the QGJFMs. To prove the mathematical theory, some color images from optical microscopy and human retina are used. Experiments and results about color image reconstruction are presented.

Obtaining regional volume changes from a deformation field is more precise when using simplex counting (SC) compared with Jacobian integration (JI) due to the numerics involved in the latter. Although SC has been proposed before, numerical properties underpinning the method and a thorough evaluation of the method against JI is missing in the literature. The contributions of this paper are: (a) we propose surface propagation (SP)—a simplification to SC that significantly reduces its computational complexity; (b) we will derive the orders of approximation of SP which can also be extended to SC. In the experiments, we will begin by empirically showing that SP is indeed nearly identical to SC, and that both methods are more stable than JI in presence of moderate to large deformation noise. Since SC and SP are identical, we consider SP as a representative of both the methods for a practical evaluation against JI. In a real application on Alzheimer’s disease neuroimaging initiative data, we show the following: (a) SP produces whole brain and medial temporal lobe atrophy numbers that are significantly better than JI at separating between normal controls and Alzheimer’s disease patients; (b) SP produces disease group atrophy differences comparable to or better than those obtained using FreeSurfer, demonstrating the validity of the obtained clinical results. Finally, in a reproducibility study, we show that the voxel-wise application of SP yields significantly lower variance when compared to JI.

We propose an adaptable framework for analyzing ultrasound (US) images quantitatively to provide computer-aided diagnosis using machine learning. Our preliminary clinical targets are hepatic steatosis, adenomyosis, and craniosynostosis. For steatosis and adenomyosis, we collected US studies from 288 and 88 patients, respectively, as well as their biopsy or magnetic resonanceconfirmed diagnosis. Radiologists identified a region of interest (ROI) on each image. We filtered the US images for various texture responses and use the pixel intensity distribution within each ROI as feature parameterizations. Our craniosynostosis dataset consisted of 22 CT-confirmed cases and 22 age-matched controls. One physician manually measured the vectors from the center of the skull to the outer cortex at every 10 deg for each image and we used the principal directions as shape features for parameterization. These parameters and the known diagnosis were used to train classifiers. Testing with cross-validation, we obtained 72.74% accuracy and 0.71 area under receiver operating characteristics curve for steatosis (p<0.0001), 77.27% and 0.77 for adenomyosis (p<0.0001), and 88.63% and 0.89 for craniosynostosis (p=0.0006). Our framework is able to detect a variety of diseases with high accuracy. We hope to include it as a routinely available support system in the clinic.

The contrast enhancement of retinal image plays a vital role for the detection of microaneurysms (MAs), which are an early sign of diabetic retinopathy disease. A retinal image contrast enhancement method has been presented to improve the MA detection technique. The success rate on low-contrast noisy retinal image analysis shows the importance of the proposed method. Overall, 587 retinal input images are tested for performance analysis. The average sensitivity and specificity are obtained as 95.94% and 99.21%, respectively. The area under curve is found as 0.932 for the receiver operating characteristics analysis. The classifications of diabetic retinopathy disease are also performed here. The experimental results show that the overall MA detection method performs better than the current state-of-the-art MA detection algorithms.

Matching the bolus arrival time (BAT) of the arterial input function (AIF) and tissue residue function (TRF) is necessary for accurate pharmacokinetic (PK) modeling of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). We investigated the sensitivity of volume transfer constant (K^trans) and extravascular extracellular volume fraction (v_e) to BAT and compared the results of four automatic BAT measurement methods in characterization of prostate and breast cancers. Variation in delay between AIF and TRF resulted in a monotonous change trend of K^trans and v_e values. The results of automatic BAT estimators for clinical data were all comparable except for one BAT estimation method. Our results indicate that inaccuracies in BAT measurement can lead to variability among DCE-MRI PK model parameters, diminish the quality of model fit, and produce fewer valid voxels in a region of interest. Although the selection of the BAT method did not affect the direction of change in the treatment assessment cohort, we suggest that BAT measurement methods must be used consistently in the course of longitudinal studies to control measurement variability.

Exposed root surfaces due to gingival recession are subject to biofilm stagnation that can result in caries formation. Cervical enamel and dentin demineralization induced by a cariogenic biofilm was evaluated using swept-source optical coherence tomography (SS-OCT). The cementoenamel junction (CEJ) sections of extracted human teeth were subjected to demineralization for 1, 2, or 3 weeks. A suspension of Streptococcus mutans was applied to form a cariogenic biofilm using an oral biofilm reactor. After incubation, demineralization was observed by SS-OCT. For the analysis of SS-OCT signal, the value of the area under the curve (AUC) of the signal profile was measured. Statistical analyses were performed with 95% level of confidence. Cervical demineralization was displayed as a bright zone in SS-OCT. The demineralization depth of dentin was significantly deeper than that of enamel (p<0.05). Enamel near the CEJ demonstrated a significant increase of AUC over the other enamel region after the demineralization. The gaps along the dentinoenamel junction were additionally observed in SS-OCT. SS-OCT was capable of monitoring the cervical demineralization induced by a cariogenic biofilm and is considered to be a promising modality for the diagnosis of cervical demineralization.

We aim at reconstructing superficial vessels of the brain. Ultimately, they will serve to guide the deformation methods to compensate for the brain shift. A pipeline for three-dimensional (3-D) vessel reconstruction using three mono-complementary metal-oxide semiconductor cameras has been developed. Vessel centerlines are manually selected in the images. Using the properties of the Hessian matrix, the centerline points are assigned direction information. For correspondence matching, a combination of methods was used. The process starts with epipolar and spatial coherence constraints (geometrical constraints), followed by relaxation labeling and an iterative filtering where the 3-D points are compared to surfaces obtained using the thin-plate spline with decreasing relaxation parameter. Finally, the points are shifted to their local centroid position. Evaluation in virtual, phantom, and experimental images, including intraoperative data from patient experiments, shows that, with appropriate camera positions, the error estimates (root-mean square error and mean error) are ∼1  mm.

Magnetic resonance-guided high intensity focused ultrasound treatment of the liver is a promising noninvasive technique for ablation of liver lesions. For the technique to be used in clinical practice, however, the issue of liver motion needs to be addressed. A subject-specific four-dimensional liver motion model is presented that is created based on registration of dynamically acquired magnetic resonance data. This model can be used for predicting the tumor motion trajectory for treatment planning and to indicate the tumor position for treatment guidance. The performance of the model was evaluated on a dynamic scan series that was not used to build the model. The method achieved an average Dice coefficient of 0.93 between the predicted and actual liver profiles and an average vessel misalignment of 3.0 mm. The model performed robustly, with a small variation in the results per subject. The results demonstrate the potential of the model to be used for MRI-guided treatment of liver lesions. Furthermore, the model can possibly be applied in other image-guided therapies, for instance radiotherapy of the liver.

Soft-tissue deformation represents a significant error source in current surgical navigation systems used for open hepatic procedures. While numerous algorithms have been proposed to rectify the tissue deformation that is encountered during open liver surgery, clinical validation of the proposed methods has been limited to surface-based metrics, and subsurface validation has largely been performed via phantom experiments. The proposed method involves the analysis of two deformation-correction algorithms for open hepatic image-guided surgery systems via subsurface targets digitized with tracked intraoperative ultrasound (iUS). Intraoperative surface digitizations were acquired via a laser range scanner and an optically tracked stylus for the purposes of computing the physical-to-image space registration and for use in retrospective deformation-correction algorithms. Upon completion of surface digitization, the organ was interrogated with a tracked iUS transducer where the iUS images and corresponding tracked locations were recorded. Mean closest-point distances between the feature contours delineated in the iUS images and corresponding three-dimensional anatomical model generated from preoperative tomograms were computed to quantify the extent to which the deformation-correction algorithms improved registration accuracy. The results for six patients, including eight anatomical targets, indicate that deformation correction can facilitate reduction in target error of ∼52%.

When searching through volumetric images [e.g., computed tomography (CT)], radiologists appear to use two different search strategies: “drilling” (restrict eye movements to a small region of the image while quickly scrolling through slices), or “scanning” (search over large areas at a given depth before moving on to the next slice). To computationally identify the type of image information that is used in these two strategies, 23 naïve observers were instructed with either “drilling” or “scanning” when searching for target T’s in 20 volumes of faux lung CTs. We computed saliency maps using both classical two-dimensional (2-D) saliency, and a three-dimensional (3-D) dynamic saliency that captures the characteristics of scrolling through slices. Comparing observers’ gaze distributions with the saliency maps showed that search strategy alters the type of saliency that attracts fixations. Drillers’ fixations aligned better with dynamic saliency and scanners with 2-D saliency. The computed saliency was greater for detected targets than for missed targets. Similar results were observed in data from 19 radiologists who searched five stacks of clinical chest CTs for lung nodules. Dynamic saliency may be superior to the 2-D saliency for detecting targets embedded in volumetric images, and thus “drilling” may be more efficient than “scanning.”

Model observers intended to predict the diagnostic performance of human observers should account for the effects of both quantum and anatomical noise. We compared the abilities of several visual-search (VS) and scanning Hotelling-type models to account for anatomical noise in a localization receiver operating characteristic (LROC) study involving simulated nuclear medicine images. Our VS observer invoked a two-stage process of search and analysis. The images featured lesions in the prostate and pelvic lymph nodes. Lesion contrast and the geometric resolution and sensitivity of the imaging collimator were the study variables. A set of anthropomorphic mathematical phantoms was imaged with an analytic projector based on eight parallel-hole collimators with different sensitivity and resolution properties. The LROC study was conducted with human observers and the channelized nonprewhitening, channelized Hotelling (CH) and VS model observers. The CH observer was applied in a “background-known-statistically” protocol while the VS observer performed a quasi-background-known-exactly task. Both of these models were applied with and without internal noise in the decision variables. A perceptual search threshold was also tested with the VS observer. The model observers without inefficiencies failed to mimic the average performance trend for the humans. The CH and VS observers with internal noise matched the humans primarily at low collimator sensitivities. With both internal noise and the search threshold, the VS observer attained quantitative agreement with the human observers. Computational efficiency is an important advantage of the VS observer.

The channelized Hotelling observer (CHO) uses the first- and second-order statistics of channel outputs under both hypotheses to compute test statistics used in binary classification tasks. If these input data deviate from a multivariate normal (MVN) distribution, the classification performance will be suboptimal compared to an ideal observer operating on the same channel outputs. We conducted a comprehensive investigation to rigorously study the validity of the MVN assumption under various kinds of background and signal variability in a realistic population of phantoms. The study was performed in the context of myocardial perfusion SPECT imaging; anatomical, uptake (intensity), and signal variability were simulated. Quantitative measures and graphical approaches applied to the outputs of each channel were used to investigate the amount and type of deviation from normality. For some types of background and signal variations, the channel outputs, under both hypotheses, were non-normal (i.e., skewed or multimodal). This indicates that, for realistic medical images in cases where there is signal or background variability, the normality of the channel outputs should be evaluated before applying a CHO. Finally, the different degrees of departure from normality of the various channels are explained in terms of violations of the central limit theorem.

Prostatic urethral transitional cell carcinoma with prostatic invasion in a dog was imaged with abdominal radiography and abdominal ultrasonography antemortem. Synchrotron in-line x-ray phase contrast imaging computed tomography (XPCI-CT) was performed on the prostate ex vivo at the Canadian Light Source Synchrotron and compared to histology. XPCI-CT imaging provides greater soft tissue contrast than conventional absorption-based x-ray imaging modalities, permitting visualization of regions of inflammatory cell infiltration, differentiation of invasive versus noninvasive tumor regions, and areas of necrosis and mineralization. This represents the first report of XPCI-CT images of an invasive prostatic urothelial neoplasm in a dog.

Biomedical phantoms are commonly used for various medical imaging modalities to improve imaging quality and procedures. Current biomedical phantoms fabricated commercially are high in cost and limited in the specificity of human environments and structures that can be mimicked. This study aimed to control the measurable computed tomography (CT) number in Hounsfield units through polymeric biomedical phantom materials using controlled amounts of hydroxyapatite (hA). The purpose was to fabricate CT phantoms capable of mimicking various coronary plaque types while introducing a fabrication technique and basis for a numerical model to which the technique may be applied. The CT number is tunable based on the controlled material properties of electron density and atomic numbers. Three different polymeric matrices of polyethylene (PE), thermoplastic polyurethane (TPU), and polyvinylidene fluoride (PVDF) were selected due to their varied specific densities and ease of fabrication acting as integral properties for CT phantom fabrication. These polymers were processed together with additions of hA in mass percentages of 2.5, 5, 10, and 20% hA as well as a 0% hA as a control for each polymeric material. By adding hA to PE, TPU, and PVDF an increasing trend was exhibited between CT number and weight percent of hA.

The open accessibility of Internet-based medical images in teleradialogy face security threats due to the nonsecured communication media. This paper discusses the spatial domain watermarking of ultrasound medical images for content authentication, tamper detection, and lossless recovery. For this purpose, the image is divided into two main parts, the region of interest (ROI) and region of noninterest (RONI). The defined ROI and its hash value are combined as watermark, lossless compressed, and embedded into the RONI part of images at pixel’s least significant bits (LSBs). The watermark lossless compression and embedding at pixel’s LSBs preserve image diagnostic and perceptual qualities. Different lossless compression techniques including Lempel-Ziv-Welch (LZW) were tested for watermark compression. The performances of these techniques were compared based on more bit reduction and compression ratio. LZW was found better than others and used in tamper detection and recovery watermarking of medical images (TDARWMI) scheme development to be used for ROI authentication, tamper detection, localization, and lossless recovery. TDARWMI performance was compared and found to be better than other watermarking schemes.

The preservation of certain labile cancer biomarkers with formaldehyde-based fixatives can be considerably affected by preanalytical factors such as quality of fixation. Currently, there are no technologies capable of quantifying a fixative’s concentration or the formation of cross-links in tissue specimens. This work examined the ability to detect formalin diffusion into a histological specimen in real time. As formaldehyde passively diffused into tissue, an ultrasound time-of-flight (TOF) shift of several nanoseconds was generated due to the distinct sound velocities of formalin and exchangeable fluid within the tissue. This signal was resolved with a developed digital acoustic interferometry algorithm, which compared the phase differential between signals and computed the absolute TOF with subnanosecond precision. The TOF was measured repeatedly across the tissue sample for several hours until diffusive equilibrium was realized. The change in TOF from 6-mm thick ex vivo human tonsil fit a single-exponential decay (R²_adj≥0.98) with rate constants that varied drastically spatially between 2 and 10 h (σ=2.9  h) due to substantial heterogeneity. This technology may prove essential to personalized cancer diagnostics by documenting and tracking biospecimen preanalytical fixation, guaranteeing their suitability for diagnostic assays, and speeding the workflow in clinical histopathology laboratories.
