On-board 4D cone-beam CT (CBCT) imaging using a linear accelerator (LINAC) is recently favored scanning protocol in image guided radiotherapy, but it raises the problems of excessive radiation dose. Alternatively, the 4D digital tomosynthesis (DTS) has been introduced for small-sized target localization, such as pancreas, prostate, or partial breast scan, which does not require a full 3D information. However, conventional filtered back-projection (FBP) reconstruction method produces severe aliasing artifacts due to sparsely sampled projections measured in each respiration phase within a limited angle range. This effect is even more severe when the LINAC gantry sweep speed is too fast to sufficiently cover the respiratory gating phase. Previous studies on total-variation (TV) minimization-based reconstruction framework would be an alternative approach to solve this problem, however, it presents the loss of sharpness during the iterations. In this study, we adopted an adaptively-weighted TV (AwTV) scheme which penalizes the images after the TV optimization. We introduced a look-up table which contains all possible weighting parameters during each iteration step. As a result, the proposed AwTV method provided better image quality compared to the conventional FBP and adaptive steepest descent-projection onto convex set (ASD-POCS) frameworks, showing higher structural similarities (SSIM) by factor of 1.12 compared to FBP and less root-mean-square error (RMSEs) by factor of 1.06 compared to ASD-POCS. The horizontal line profile of the spherical target inserted in the moving phantom showed that the images from FBP and ASD-POCS provided severe aliasing artifact and smoothed pixel intensities, but proposed AwTV scheme reduces the aliasing artifact while maintaining the object’s sharpness. In conclusion, the proposed AwTV method has a potential for low-dose and faster 4D-DTS imaging, which indicates an alternative option to 4D-CBCT for small region target localization.

Quantitative imaging analysis has become a focus of medical imaging fields in recent days. In this study, Fourier-based imaging metrics for task-based quantitative assessment of lung nodules were applied in low-dose chest tomosynthesis. Compared to the conventional filtered back-projection (FBP), a compressed-sensing (CS) image reconstruction has been proposed for dose and artifact reduction. We implemented the CS-based low-dose reconstruction scheme to a sparsely sampled projection dataset and compared the lung nodule detectability index (d’) between the FBP and CS methods. We used the non-prewhitening (NPW) model observer to estimate the in-plane slice detectability in tomosynthesis and theoretically calculated d’ using the weighted amounts of local noise, spatial resolution, and task function in Fourier domain. We considered spatially varying noise and spatial resolution properties because the iterative reconstruction showed non-stationary characteristics. For analysis of task function, we adopted a simple binary hypothesis-testing model which discriminates outer and inner region of the encapsulated shape of lung nodule. The results indicated that the local noise power spectrum showed smaller intensities with increasing the number of projections, whereas the local transfer function provided similar appearances between the FBP and CS schemes. The resulted task functions for the same size of lung nodules showed the same pattern with different intensity, whereas the task function for different size of lung nodules presented different shapes due to different object functions. The theoretically calculated d’ values showed that the CS schemes provided higher values than the FBP method by factors of 2.64-3.47 and 2.50-3.10 for two different lung nodules among all projection views. This could demonstrate that the low-dose CS algorithm provide a comparable lung nodule images in comparison to FBP from 37.9% up to 28.8% reduced dose in the same projection views. Moreover, we observed that the CS method implemented with small number of projections provided similar or somewhat higher d’ values compared to the FBP method with large number of projections. In conclusion, the CS scheme may present a potential dose reduction for lung nodule detection in the chest tomosynthesis by showing higher d’ in comparison to the conventional FBP method.

Patient respiration induces motion artifacts during cone-beam CT (CBCT) imaging in LINAC-based radiotherapy guidance. This could be relieved by retrospectively sorting the projection images, which could be called as a respiratorycorrelated CBCT or a four-dimensional (4D) CBCT imaging. However, the slowness of the LINAC head gantry rotation limits a rapid scan time so that 4D-CBCT usually involves large amounts of radiation dose. Digital tomosynthesis (DTS) which utilizes limited angular range would present a faster 4D imaging with much lower radiation dose. One drawback of 4D-DTS is strong streak artifacts represented in the tomosynthesis reconstructed images due to sparsely sampled projections in each phase bin. The authors suggest a fast low-dose 4D-DTS image reconstruction method in order to effectively reduce the artifacts in a sparse imaging tomosynthesis condition. We used a flat-panel detector to acquire tomosynthesis projections of respiratory moving phantom in anterior-posterior (AP) and lateral views. We entered a sinusoidal periodic respiratory motion for an input signal to the phantom. An external monitor of Varian real-time position management (RPM) system was used to estimate the input respiratory motion, thereby four respiratory gating phases were determined to retrospectively arrange the projections. For streak line reduction, we introduced a simple iterative scheme suggested by McKinnon and Bates (MKB) and regarded it as a prior input image of the proposed lowdose compressed sensing (CS) method. Three different 4D-DTS image reconstruction schemes of conventional Feldkamp (FDK), MKB, and MKB-CS were implemented to phase-wise projections of both AP and lateral views. All reconstructions were accelerated by using a GPU card to reduce the computation times. For assessment of algorithmic performance, we compared a streak reduction ratio (SRR) and a contrast-to-noise-ratio (CNR) among the outcome images. The results showed that SRRs for MKB and MKB-CS schemes were 0.24 and 0.69, respectively, which indicates that the proposed MKB-CS method provided smaller streaking artifacts than conventional one by factor of 2.88. The resulted CNRs of coronal tomosynthesis images at peak-inhale phase were 3.24, 6.36, and 10.56 for FDK, MKB, and MKB-CS, respectively. This shows that the proposed method provides better image quality compared to others. The reconstruction time for MKB-CS was 196.07 sec, showing that our GPU-accelerated programming would enhance the algorithmic performance to match clinically feasible times (~3 min). In conclusion, the proposed low-dose 4D-DTS reconstruction scheme may provide better outcomes than the conventional methods with fast speed, and could thus it could be applied in practical 4D imaging for radiotherapy.

Lung cancer is the leading cause of cancer death worldwide and the early diagnosis of lung cancer has recently become more important. For early screening lung cancer, computed tomography (CT) has been used as a gold standard for early diagnosis of lung cancer [1]. The major advantage of CT is that it is not susceptible to the problem of misdiagnosis caused by anatomical overlapping while CT has extremely high radiation dose and cost compared to chest radiography. Chest digital tomosynthesis (CDT) is a recently introduced new modality for lung cancer screening with relatively low radiation dose compared to CT [2] and also showing high sensitivity and specificity to prevent anatomical overlapping occurred in chest radiography. Dual energy material decomposition method has been proposed for better detection of pulmonary nodules as means of reducing the anatomical noise [3]. In this study, possibility of material decomposition in CDT was tested by simulation study and actual experiment using prototype CDT. Furthermore organ absorbed dose and effective dose were compared with single energy CDT. The Gate v6 (Geant4 application for tomographic emission), and TASMIP (Tungsten anode spectral model using the interpolating polynomial) code were used for simulation study and simulated cylinder shape phantom consisted of 4 inner beads which were filled with spine, rib, muscle and lung equivalent materials. The patient dose was estimated by PCXMC 1.5 Monte Carlo simulation tool [4]. The tomosynthesis scan was performed with a linear movement and 21 projection images were obtained over 30 degree of angular range with 1.5° degree of angular interval. The proto type CDT system has same geometry with simulation study and composed of E7869X (Toshiba, Japan) x-ray tube and FDX3543RPW (Toshiba, Japan) detector. The result images showed that reconstructed with dual energy clearly visualize lung filed by removing unnecessary bony structure. Furthermore, dual energy CDT could enhance spine bone hidden by heart effectively. The effective dose in dual energy CDT was slightly higher than single energy CDT, while only 10% of average thoracic CT [5]. Dual energy tomosynthesis is a new technique; therefore, there is little guidance for its integration into the clinical practice and this study can be used to improve diagnosis efficiency of lung field screening using CDT

In conventional digital radiography (DR) using a dual energy subtraction technique, a significant fraction of the detected photons are scattered within the body, resulting in the scatter component. Scattered radiation can significantly deteriorate image quality in diagnostic X-ray imaging systems. Various methods of scatter correction, including both measurement and non-measurement-based methods have been proposed in the past. Both methods can reduce scatter artifacts in images. However, non-measurement-based methods require a homogeneous object and have insufficient scatter component correction. Therefore, we employed a measurement-based method to correct for the scatter component of inhomogeneous objects from dual energy DR (DEDR) images. We performed a simulation study using a Monte Carlo simulation with a primary modulator, which is a measurement-based method for the DEDR system. The primary modulator, which has a checkerboard pattern, was used to modulate primary radiation. Cylindrical phantoms of variable size were used to quantify imaging performance. For scatter estimation, we used Discrete Fourier Transform filtering. The primary modulation method was evaluated using a cylindrical phantom in the DEDR system. The scatter components were accurately removed using a primary modulator. When the results acquired with scatter correction and without correction were compared, the average contrast-to-noise ratio (CNR) with the correction was 1.35 times higher than that obtained without correction, and the average root mean square error (RMSE) with the correction was 38.00% better than that without correction. In the subtraction study, the average CNR with correction was 2.04 (aluminum subtraction) and 1.38 (polymethyl methacrylate (PMMA) subtraction) times higher than that obtained without the correction. The analysis demonstrated the accuracy of scatter correction and the improvement of image quality using a primary modulator and showed the feasibility of introducing the primary modulation technique into dual energy subtraction. Therefore, we suggest that the scatter correction method with a primary modulator is useful for the DEDR system.

Photon counting detector based on semiconductor materials is a promising imaging modality and provides many benefits for x-ray imaging compared with conventional detectors. This detector is able to measure the x-ray photon energy deposited by each event and provide the x-ray spectrum formed by detected photon. Recently, photon counting detectors have been developed for x-ray imaging. However, there has not been done many works for developing the novel x-ray imaging techniques and evaluating the image quality in x-ray system based on photon counting detectors. In this study, we simulated computed tomography (CT) images using projection-based and image-based energy weighting techniques and evaluate the effect of energy weighting in CT images. We designed the x-ray CT system equipped with cadmium telluride (CdTe) detector operating in the photon counting mode using Geant4 Application for Tomographic Emission (GATE) simulation. A micro focus X-ray source was modeled to reduce the flux of photons and minimize the spectral distortion. The phantom had a cylindrical shape of 30 mm diameter and consisted of ploymethylmethacrylate (PMMA) which includes the blood (1.06 g/cm³), iodine, and gadolinium (50 mg/cm³). The reconstructed images of phantom were acquired with projection-based and image-based energy weighting techniques. To evaluate the image quality, the contrast-to-noise ratio (CNR) is calculated as a function of the number of energy-bins. The CNR of both images acquired with energy weighting techniques were improved compared with those of integrating and counting images and increased as a function of the number of energy-bins. When the number of energy-bins was increased, the CNR in the image-based energy weighting image is higher than the projection-based energy weighting image. The results of this study show that the energy weighting techniques based on the photon counting detector can improve the image quality and the number of energy-bins used for generating the image is important.

The single photon emission computed tomography (SPECT) based on pixelated Cadmium Telluride (CdTe) detectors has been promising for high resolution small animal imaging. For the feasibility of our CdTe detector (PID350, AJAT, Finland) in small animal SPECT system, we compared the quality of reconstructed images from our SPECT system to those from a conventional small animal SPECT system with a NaI(Tl) detector. The CdTe detector had 0.35 mm × 0.35 mm² pixels, which is similar to the size of the available detector (PID350, AJAT, Finland). The CdTe detector size was 44.8 × 44.8 mm² and the thickness of this detector was 1 mm. The size of NaI(Tl) detector was 44 mm × 96 mm² and the thickness of this detector was 5 mm. The intrinsic spatial resolution of CdTe detector and that of NaI(Tl) detector were 0.35 mm FWHM and 2.3 mm FWHM, respectively. The parallel hole collimator of lead was 25 mm high, and the septum was 0.2 mm high. The parallel hole collimator was hexagonal with a 0.5 mm radius. The Monte Carlo simulations were performed to assess both SPECT systems. All projections were obtained using 120 projection views acquired from 0° to 360° with a 3° step. Slices were reconstructed using an Ordered Subsets Expectation Maximization (OS-EM) with 5 iterations and 4 subsets. We compared the quality of the images from each system in terms of spatial resolution (FWHM), and sensitivity. The spatial resolution of system using CdTe and that of system using NaI(Tl) detector were 2.32 mm FWHM and 3.31mm FWHM, respectively. The system sensitivity for our SPECT with CdTe and NaI(Tl) detector were 23.22 count/sec/MBq and 54.06 count/sec/MBq, respectively. Our simulation results indicated that our SPECT with CdTe detector showed better spatial resolution compared to SPECT with NaI(Tl) detector. These results demonstrate that SPECT imaging using a pixelated CdTe detector can improve the performance of SPECT systems for small animal imaging in term of spatial resolution.

Yonsei University Medical Center (YUMC) in Seoul, Korea is 114 years old and 1,582 beds in Shinchon Severance hospital in main university campus and 746 beds in affiliated Youngdong Severance hospital which is 20 miles away from the main campus. The dental hospital in main campus is also included in a full-PACS system. The numbers of exams/year for main, affiliated, and dental hospitals are approximately 558,000, 365,000, and 181,000, respectively. Since 1997, a Mini-PACS with 3xMRI, 2xDSI, and 2xCT in Shinchon Severance hospital has been operating to archive the digital data and to view them with DICOM viewer PiView^TM. An archiving system with 2xCT and 2xMRI in Youngdong Severance has been operating to archive the digital data. We are now designing a large-scale full-PACS for YUMC with experiences of running a mini-PACS for 3 years. The 11xUS, 7xEndoscopy, 7xCR, 3xSPECT, 1xPET, 1xCT simulator, and digital camera based patient database in the Dept. of radiation oncology in Shinchon Severance hospital will be connected to an archiving server system through modality interface gateway. The 3xCR, 2xDSA, 2xFD, 5xUS, 3xEndoscopy in Youngdong Severance hospital will be connected to the main archiving system. The 1xCT, 10xIntraoral X-ray unit DR, 4xPanoramic&cephalometric unit DR, 1xTranscranial CR, 1xScanora X-ray unit CR, 1xSectography CR in dental hospital will be connected to archiving server system through modality interface gateway. The estimated amount of data for Shinchon severance, Youngdong severance, and dental hospitals per year are 11.55TB, 5.88TB, and 0.96TB, respectively. The current mini-PACS server includes 54 GB RAID, 520 GB DLT with SUN Spectra^TM server. The main server in Shinchon Severance hospital needs to be upgraded to 600GB RAID for 30 days and 10TB ODJ or DLT for the first two years. Youngdong Severance hospital needs to be installed a main server with 600GB RAID for > 30 days and 10TB ODJ or DLT for > 2 years. The interface between HIS/RIS and PACS needs to be developed using a PACS broker. YUMC has successfully been operating the Mini- PACS for last 3 years and now expanding to a full-PACS for 558,000 exams/year in Shinchon Severance, 365,000 exams/year in affiliated Youngdong Severance, and 181,000 exams/year in dental hospitals.
